MS CRM 2013 "Your changes have not been saved..." message is not showing in the dailog

Hello,

Since upgrading from CRM 2011 to 2013, our organization has been experiencing issues
with the below dialog box. the below are the steps which we are following.

1. Navigate the entity from the "Workplace"
2. Select the entity then
3. Open the record and change the field data and then
4. Navigate to the Entity 

It is showing empty dialog instead of the dialog with message "Your changes have not
been saved....."
Any help greatly appreciated....
